Let us know you’re ready to be a Bobcat by submitting your confirmation deposit.
The following checklist will assure a smooth transition to FSU. It has everything you need to get started! To complete the following forms you will need:
- Student ID
- Username
- Password
Your Student ID was included in your acceptance email. You can visit the Password Management page for information on your FSU username and password. Check your email for a message from newfsuaccount@agrovidaarin.com.
The forms listed below will help you complete your enrollment at Frostburg State University.
Pay your deposit (required)
Let us know you want to become a Frostburg State Bobcat by paying your deposit! You can pay your deposit online.
If paying your deposit online, be sure to have your FSU Username and Password accessible.
Note: All full-time, first-year FSU students are required to complete one of two applications: our application to receive housing OR the application for exemption to the residency requirement.
Contact the Residence Life Office for Residence Hall information or Edgewood Commons for apartment information.
Complete and return your medical forms (required)
All forms must be completed through the Brady Health Patient Portal, once you have received your FSU username and password. Due date for requirements is August 1st for Fall semester and January 2nd for Spring semester. *The required records MUST BE scanned and uploaded through the Patient Portal. Our system is all electronic. NO mailed, fax or email copies of immunization records are accepted.*
Contact Brady Health Center by email or call 301.687.4310 during normal business hours if you need assistance.
- Upload Immunization Record through the Brady Health Patient Portal, once you have received your FSU username and password.
- Complete TB Screen (Tuberculosis Risk Assessment Form Questionnaire) Addition information (access Brady Health Patient Portal)
- Consent to Treat Minor Form (Parent/Guardian signature required if the student will be under 18 at date of first attendance) (Brady Health Patient Portal)
- Review important Meningitis On Campus Information
Submit the Disability Support Services Form
Frostburg State University and The Student Accessibility Services Office (SASO) are committed to providing equal access to all University facilities, programs, and activities for students with disabilities. To be fully registered with the office and receive accommodations, please complete the following:
- Fill out the Accessibility Application Request Form online.
- Please send this link to your medical provider: Provider Portal (If provided medical documentation by your provider, please email it to disability@agrovidaarin.com using jpeg or pdf)
- Medical documentation or psychological (or educational) evaluation (Please upload if you have documentation already.)
- If applying for an Emotional Support Animal, use this link. (You still must complete #2 and #3)
All documentation needs to be completed and turned in for the process to be completed to become registered with the Student Accessibility Services Office. Please note we do not accept 504/IEP forms by themselves; however, they can be included as supplemental documentation. If you have further questions, please do not hesitate to click on “New Students” link.

Attend Bobcat Bound Day
Step into Frostburg’s academic and student life by meeting faculty, staff, and students in your area. All incoming students are required to attend a Bobcat Bound Day new student orientation. You will receive your fall course schedule and get your official Frostburg Student ID card!

Grab your textbooks!
Now that you have your schedule, you can easily order your textbooks through our University Store! Use the easy textbook tool to select your term and enter your classes. It will find all your books for you! Don’t forget to grab some FSU gear while you’re there!
